THRIVE Program Meeting Request

Through the THRIVE Program, our team of Community Resource Coordinators (CRC) can provide intensive case management by assessing your strengths and needs, making referrals, helping you develop goals, and coaching you on your path to financial stability. You should receive a response within five business days of submitting a request online. Please do not submit multiple requests.

  • Bank Statement
  • 3 Weeks of Pay Stubs
  • SSI/Disability Income Statement
  • Any other Proof of Income
  • Proof of Mailing Address
  • Proof of Employment and Wages

Please note: Our THRIVE team does NOT provide emergency assistance. If you are seeking urgent financial assistance for rent, utilities, etc., we encourage you to reach out to 2-1-1 to see what services might be available in our community to help.

The tracking system is a shared computerized record-keeping system that captures information about people experiencing need for emergency services. I understand that all information gathered about me is personal and private and that I do not have to participate in the tracking system. I have had an opportunity to ask questions and to review the basic identity information, which is authorized by the release for the services provided to me by agencies participating in the tracking system. I understand that information about non-confidential services provided to me by participating agencies may be shared with other participating agencies. This release of information will be effective for 3 years from today unless I make a formal request to the agency to no longer participate in the tracking system. All information is kept confidential within the United Way system.(Required)
